Job summary

DFW Home Health is seeking a full-time Home Health Schedule Specialist in Arlington, Texas. This office-based role runs Monday through Friday from 8:00am to 4:30pm, coordinating patient referrals and visits using an online scheduling system.

You will collaborate with the Team Leader to match clinicians to patient needs, monitor referrals, and communicate with field staff to resolve scheduling issues while delivering quality patient care and service.

Qualifications

• High school education or equivalent required.
• Preferred: 1+ year scheduling experience in healthcare using online scheduling systems.
• Excellent organizational, customer service, communication, and decision-making skills.
• Knowledge of state and federal regulations governing OASIS visits, supervisory, and reassessment visits.

Responsibilities

• Uses an automated scheduling system to maintain a calendar of services for episodic and per-visit visits.
• Processes workflow for scheduled, missed, rescheduled, reassigned, declined, and delivered visits.
• Monitors pending referrals daily and assigns licensed professionals and case managers for start-of-care visits.
• Communicates daily with field staff about visits that are late, pending, or incomplete to resolve issues.
