Base pay range
$23.00/hr - $36.50/hr

PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS:
- Responsible for maintaining ongoing and accurate communication with Team Member and Manager of Clinical Practice regarding patient visit schedules.
- Assures timely completion of individual Team Member schedules within established time parameters and in consideration of Team Member productivity expectations.
- Accesses patient and clinician schedules, authorizations, and physician orders to determine appropriate visit schedules under the direction of the Manager of Clinical Practice.
- Assures efficient use of Team Member by utilizing full-time Team Member prior to scheduling part-time and per diem Team Member.
- Works under the direction of the Manager of Clinical Practice to rearrange schedules in situations of conflict or high utilization of service.
- Ensures prompt scheduling of emergent/as-needed visits as directed by the Manager of Clinical Practice.
- Coordinates, assigns, and notifies appropriate on-call Team Member and management of Weekend/Holiday schedule under the direction of management.
- Demonstrates high degree of commitment to customer service excellence.
- Exhibits exemplary interpersonal relationship skills, written, and verbal communications.
- Other responsibilities as assigned by the Manager of Clinical Practice.
QUALIFICATIONS:
- Three to Five years’ relevant experience - required
- Home Health experience - required
- HCHB experience - preferred
- Demonstrated organizational skills.
- Excellent interpersonal skills.
- Computer proficiency.
- Excellent verbal and written communication skills
