Job summary

Amedisys is seeking a Registered Nurse for a full-time home health role in New Castle County, Delaware. You will assess patients, develop and implement care plans, supervise LPNs and HHAs, and coordinate with the care team to promote patient independence at home.

Qualifications

• 1+ year of clinical RN experience.
• Current RN license for the assigned state.
• Current CPR certification and valid driver's license.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Supervises LPNs and HHAs and communicates patient progress to the clinical team.
• Completes timely and accurate documentation and participates in continuing education.
• On-call responsibilities as assigned.
