Job summary

Cooperative Home Care is hiring an RN Visit Nurse to work across lines of business, primarily Home Health. The role includes using a company car and supports a positive company culture. You will work one-on-one with clients outside of facilities, delivering compassionate, patient-centered care.

Key responsibilities include monitoring patients, coordinating with physicians, and supervising LPNs. A valid RN license and 1 year RN experience are preferred; Home Health experience is a plus.

Qualifications

Responsibilities

• Monitor patients’ condition and assess needs to provide optimal care.
• Observe and interpret symptoms; communicate findings to physicians.
• Collaborate with physicians and nurses to create individualized care plans.
• Perform routine procedures and update patient charts.
• Administer medications per physician orders; provide treatments.
• Maintain hygiene and safety; decontaminate equipment and surfaces.
• Provide instant medical care in emergencies.
• Supervise and train LPNs and nursing assistants.
• Foster a compassionate environment for patients and families.
• Attend educational workshops and conferences to expand skills.

Cooperative Home Care is hiring an RN Visit Nurse to work with all our lines of business, but primarily Home Health, effective immediately. Most of our patients are dependent on the care they receive from our RNs and caregivers, so we hire the most compassionate and thoughtful talent in the field. This position includes the use of a company car, as well as the support of a positive company culture. If you enjoy working one-on-one with clients, outside of a facility or hospital setting.

Responsibilities

• Monitor patients’ condition and assess their needs to provide the best possible care and advice.
• Observe and interpret patient’s symptoms and communicate them to physicians.
• Collaborate with physicians and nurses to devise individualized care plans for patients.
• Perform routine procedures (blood pressure measurements, administering injections etc.) and fill in patient’s charts.
• Adjust and administer patient’s medication and provide treatments according to physician’s orders.
• Act to maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing bed, etc.).
• Provide instant medical care in emergencies.
• Supervise and train LPNs and nursing assistants.
• Foster a supportive and compassionate environment to care for patients and their families.
• Expand knowledge and capabilities by attending educational workshops, conferences etc.

Skills

• Valid nursing license.
• Proven experience as a Registered Nurse, 1 Year of experience preferred
• Home Health experience preferred.
• Excellent knowledge of nursing care methods and procedures.
• In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and adherence to these policies.
• A team player with excellent communication and interpersonal skills.
• Responsible and compassionate.
• Outstanding organizational and multi-tasking skills.
• Patience with excellent problem-solving skills.

Salary: $80,000 - $90,000/year
