Job summary

Attend Home Care is seeking a compassionate Registered Nurse (RN) for Part-Time/Per-Diem roles in home health. The RN will perform Start of Care visits, develop care plans, and deliver skilled nursing services in patients' homes.

The position offers flexible scheduling and opportunities for growth. Ideal candidates hold an active RN license, with experience in home health or similar settings, and strong clinical assessment abilities.

Responsibilities

• Conduct SOC visits to complete comprehensive clinical assessments and develop physician-directed care plans.
• Perform skilled nursing visits and ongoing reassessments and adjust care plans as needed.
• Provide hands-on skilled nursing care including wound care, meds, injections, and chronic disease management.
• Educate patients and caregivers on diagnoses, medications, and preventive care to reduce hospitalizations.
• Collaborate with physicians and interdisciplinary team to ensure coordinated, high-quality care.

Part-Time/ Per-Diem (Flexible Scheduling Available)

Compensation

Competitive Pay starting for PRN: $125 SOC Visit / $75 Reassessment Visit + Mileage

What You Will Do

• Conduct Start of Care (SOC) visits to complete comprehensive clinical assessments and develop individualized, physician-directed care plans
• Perform skilled nursing visits and ongoing reassessments to monitor patient condition and adjust care plans as needed
• Provide hands-on skilled nursing care , including wound care, medication management, injections, and chronic disease management
• Deliver patient and caregiver education on diagnoses, medications, and preventive care to reduce hospitalizations
• Collaborate with physicians and interdisciplinary team members to ensure coordinated, high-quality care
• Maintain accurate and timely clinical documentation in compliance with state regulations and agency standards
• Supervise and delegate care to LPNs and Home Health Aides to ensure quality and adherence to the plan of care

What You Will Have

• Active Registered Nurse (RN) license in the state of practice
• Experience in home health, skilled nursing, or a related healthcare setting preferred
• Strong clinical assessment and critical thinking skills
• Knowledge of home health regulations and documentation standards
• Excellent communication and interpersonal skills
• Ability to work independently and manage time effectively
