You will assess, plan, and provide personalized nursing services in patients' homes, collaborating with families and physicians to optimize care. Ideal candidates hold an RN license, CPR certification, and a patient-first mindset with excellent communication skills.

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing program.
• Current state RN license.
• Proof of CPR certification.
• Compassionate, patient-first mindset.
• Excellent verbal and written communication skills.
• Current driver’s license and reliable transportation.
• Ability to work a flexible schedule and travel locally.

Responsibilities

• Visit patients in their homes for personalized, one-on-one care visits.
• Complete initial and ongoing assessments to identify patient needs.
• Assess patients’ conditions, chart observations, and perform evaluation tasks including vital signs and medication review.
• Administer medications as prescribed by the physician.
• Educate patients and families on home health care strategies and device usage where applicable.
• Provide clear communication to patients, families, and other healthcare professionals.

Responsibilities

As a Home Health RN, you will:

• Visit patients in their homes for personalized, one-on-one care visits
• Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients
• Assess patients’ conditions, chart observations, and perform evaluation tasks including vital signs and medication review
• Administer medications as prescribed by the patient’s physician
• Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient’s quality of life
• Provide effective communication to patients, family members, team members, and other healthcare professionals
