You will coordinate total care plans with physicians and healthcare teams to sustain continuity of care and meet payer requirements.

Qualificações

• Current and unrestricted RN licensure in the state of practice.
• Valid driver's license and access to reliable transportation.

Responsabilidades

• Provide high-quality clinical nursing services within scope of practice.
• Coordinate care from admission to discharge with the care team.
• Complete nursing assessments per program requirements.
• Ensure eligibility and medical necessity for services as defined by payer.
