Qualifications

• BSN preferred.
• Current license to practice professional nursing in IL.
• CPR certification required.
• Valid IL driver’s license (MVR) required per policy.
• Minimum two years of home health experience.
• Supervisory experience preferred.
• Participation in Memorial Health Leadership Development Institute.

Responsibilities

• Plans and coordinates program development for Home Health.
• Directly responsible for day-to-day operations of the clinical team.
• Oversees orientation, supervision and evaluation of clinical staff.
• Provides direct patient care of patients of all ages as needed.
• Maintains 24-hour accountability and responsibility for patient care provided by the team.
• Promotes staff development and education.
• Keeps Clinical Manager and Director informed of operations.
