Job summary

Talented Medical Solutions is seeking a Registered Nurse for Home Health to deliver skilled nursing care in patients' homes. You will assess, treat and coordinate care across an assigned caseload, updating plans of care with physicians and the interdisciplinary team.

You will educate patients and caregivers for safe self-management, perform IV therapies, wound care and medication management, and collaborate with therapists, social workers and home health aides. On-call may be required.

Qualifications

• Must be a graduate of an accredited nursing program.
• RN licensure in the state of practice is required.
• BLS certification and a valid driver's license are required.

Responsibilities

• Conduct comprehensive admission and ongoing assessments in the patient's home.
• Develop, implement and revise the plan of care in collaboration with the physician.
• Perform skilled nursing interventions, including wound care, medication management and IV therapy.
• Educate patients and caregivers on disease management, medications and safety in the home.
• Coordinate care with therapy, social work, home health aides and community resources.
• Complete timely, accurate documentation and participate in on-call coverage as scheduled.
