Job summary

UP Health System is seeking a Registered Nurse for Home Health to deliver direct patient care in home settings and coordinate plan of care with physicians and care teams.

Responsibilities include assessments, documentation, and collaboration with families to ensure quality outcomes. Requires active RN license; home health experience preferred and CPR certification. Sign-on bonus of $10,000 is offered.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current CPR certification or ability to obtain within 90 days of hire.
• 1+ years of Home Health preferred or equivalent.
• Excellent communication and organizational skills.

Responsibilities

• Provide nursing care to patients in their homes as prescribed by the physician and compliant with laws and agency policies.
• Coordinate total plan of care with other health care professionals and patients’ families.
• Perform assessments (OASIS), medication reconciliation, and thorough documentation.
• Participate in on-call and weekend rotations as needed to meet patient needs.
