You will coordinate multidisciplinary care, perform visits, and ensure accurate OASIS data collection, with physician and office collaboration for optimal patient outcomes.

Qualifications

• Must be licensed Registered Nurse in the state of employment.
• Minimum of one year of clinical experience preferred.
• Demonstrate knowledge and skill in current nursing practice.
• Must possess a valid state driver’s license and automobile liability insurance.

Responsibilities

• Perform visits and coordinate client care.
• Act as point of contact for all disciplines involved with patient care.
• Consult with physician and office on patient care details.
• Perform accurate OASIS data collection.
• Ensure medication profile is current and labs are reported timely.
• Attend weekly case conferences and monthly case manager meetings.
