Job summary

UnitedHealth Group is seeking a Registered Nurse in Home Health to provide and direct nursing care to patients in their homes as prescribed by the physician, coordinating the plan of care with the care team to ensure seamless, high-quality outcomes.

The position includes a $3,000 sign-on bonus for external candidates, with compensation described as total cash that combines per-visit pay and hourly earnings, based on productivity and local market conditions.

Qualifications

• Active RN license in state of practice.
• Minimum 1+ year Home Health experience.
• Valid driver’s license and reliable transportation.
• Current CPR certification.
• Ability to work in diverse home settings.

Responsibilities

• Develop and revise the plan of care with physician and health team.
• Supervise home health aides and licensed nurses; provide instruction and assign tasks.
• Provide supervisory visits and ensure compliance with regulations.
• Complete assessments (OASIS), medication reconciliation, and timely documentation.
• Document visits per policy and payer requirements; coordinate with care team.
• Communicate with physicians, patients, and families to ensure quality care.
• Participate in on-call and weekend rotations; support survey readiness.
