The University of Tennessee Home Health in Morristown, TN is seeking a Registered Nurse to join our Home Health team. The RN provides and directs nursing care in patients' homes, per physician orders and agency policies.

You will evaluate patients, develop and implement care plans, document progress, and collaborate with physicians and families. A valid TN RN license and CPR are required; reliable transportation is essential. This is a full-time position with comprehensive benefits.

Formación

• One year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

Responsabilidades

• Coordinate the total plan of care with other health care professionals involved in the care and strive to maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources.
• Make the initial and/or comprehensive nursing evaluation visit, determine primary focus of care, develop the plan of care within state specific guidelines, and ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer.
• Maintain and submit accurate documentation and clinical notes, including the patient's condition, appropriate goals, interventions, and patient's response to home health care.
• Provide high-quality clinical services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, from admit through discharge.
