Elara Caring is hiring a Registered Nurse Home Health to deliver Right Care at the Right Time in patients' homes. You will assess needs, complete ongoing assessments, and provide nursing care per physician orders while safeguarding privacy and patient safety.

Responsibilities include monitoring conditions, educating families, and acting on changes to ensure high-quality care. Travel between patient homes is required with a valid license and reliable transportation.

Formación

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must possess a valid driver's license or reliable transportation.

Responsabilidades

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy per HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.
