Elara Caring seeks a Registered Nurse Home Health to assess patients, update plans of care, and deliver nursing services in patients’ homes. You will work with the care team to ensure timely, high-quality home health care.

The role requires an active RN license, 1+ year clinical nursing experience (home health preferred), and the ability to lift up to 50 pounds with reliable transportation for travel between patient homes.

Qualifications

• Current State license as a Registered Nurse (RN).
• Graduate of an accredited nursing program (Certificate, Diploma, ADN or BSN).
• 1+ year of nursing experience in a clinical care setting.
• Home Health experience is preferred but not required.
• Ability to lift up to 50 pounds and to transport/position patients.
• Must have a valid driver's license or reliable transportation to travel to patient homes.

Responsabilités

• Assess patients to determine care needs and complete initial and ongoing assessments.
• Protect patient and staff privacy in accordance with HIPAA.
• Provide nursing care according to the plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families on care and safety.
• Take appropriate action when patient conditions change.
• Help ensure quality, safe, and continuous patient care.
