Job summary

Halcyon Home LLC is seeking a dedicated Home Health Registered Nurse for PRN visits in Irving, Texas. You will assess patients, implement and adjust plans of care, and educate families to support aging patients in their homes. The role requires strong clinical skills, independence, and compassionate communication.

Qualifications

Current RN license and good standing with the state nursing board.
Experience in home health or related healthcare setting preferred.
Strong clinical and assessment skills.
Excellent communication and interpersonal abilities.
Ability to work independently and manage time effectively.

Responsibilities

Perform initial and ongoing comprehensive assessments of patient needs.
Develop and revise the plan of care to ensure quality and continuity of care.
Educate patients and families on disease processes, self-care, and end-of-life decisions.
Coordinate with the IDG to prioritize patient needs and provide caregiver training as needed.
Deliver nursing care with infection control and standard precautions.
Provide clinical direction to Hospice Aide and LPN/LVN as required.
Document progress and communicate changes to the physician and care team.
