Halcyon Home is seeking a dedicated Home Health Registered Nurse PRN Pay Per Visit to support our geriatric population in Irving, TX, and surrounding areas. We offer competitive pay and strong team support.

Qualifications

Current RN license? (license in good standing)
Experience in home health or related healthcare setting preferred
Strong clinical and assessment skills
Excellent communication and interpersonal abilities
Ability to work independently and manage time effectively

Responsibilities

Conduct initial and ongoing comprehensive assessments of patient needs
Determine appropriate levels of care and eligibility for services
Develop and update the plan of care for quality and continuity
Educate patients and families on disease processes, self-care, and end-of-life care
Coordinate with the IDG and provide staff training as needed
Assess acuity to determine service frequency and caregiver readiness
Provide infection control and standard precautions in patient care
Guide Hospice Aide and LPN/LVN to ensure service continuity
Document progress notes showing goal achievement and outcomes
Communicate patient changes to physicians and team members
Monitor cases for third-party payer requirements
Demonstrate professionalism and promote agency policies
