Job summary

Halcyon Home LLC Fort Worth, TX seeks a compassionate Home Health Registered Nurse PRN Pay Per Visit to support geriatric patients in Irving and surrounding areas. You will perform initial and ongoing assessments, develop and implement care plans, and educate patients and families about end-of-life care and self-care.

Applicants must hold an active RN license, have home health experience or related healthcare background, and strong communication and time-management skills.

Qualifications

• Current RN license and good standing with the state nursing board.
• Experience in home health or related healthcare setting preferred.
• Strong clinical and assessment skills; excellent communication and interpersonal abilities.
• Ability to work independently and manage time effectively.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs and condition.
• Develop and implement the plan of care to ensure quality and continuity of care.
• Educate patient and family about disease process, end-of-life care, self-care techniques.
• Provide clinical directions to home health aides and LPNs/LVNs as needed.
• Prepare clinical notes and communicate changes in patient needs to the physician.
• Monitor cases for third-party payor requirements and ensure compliant care delivery.
• Promote agency policies and effective communication with the care team.
