Job summary

Beth Israel Lahey Health at Home in Beverly, MA is seeking an experienced registered nurse for per diem day shifts to plan, coordinate, and deliver home care services. You will assess patient needs, initiate and revise care plans, administer medications, and educate families within regulatory guidelines.

You'll supervise home health aides biweekly, ensure HIPAA compliance, document in EPIC, and support discharge planning while leveraging community resources to improve outcomes.

Qualifications

• Active Massachusetts RN license.
• American Heart Association BLS certification.
• OASIS and EPIC experience preferred.
• At least 1 year of direct care nursing experience.

Responsibilities

• Complete and document an initial assessment of patient and family to determine home care needs.
• Initiates the plan of care and revises it as patient status changes.
• Administers medications and treatments as prescribed by the physician.
• Instruct, supervise, and evaluate the home health aide care provided every two weeks.
• Identify discharge planning needs as part of the care plan development.
• Practice confidentiality and HIPAA guidelines; complete OASIS in EPIC.
