Responsibilities

• Conduct comprehensive patient assessments including vitals and health status.
• Develop individualized care plans with medication administration, wound care and disease management.
• Educate patients and families on disease management and self‑care.
• Provide direct care including wound care, injections, and IV therapy.
• Collaborate with physicians and therapists for coordinated care.
• Maintain accurate documentation of assessments, interventions and progress.
• Adhere to HIPAA and Medicare regulations.
