Millennium Physician Group seeks a dedicated Registered Nurse (RN) to join our Home Health Team in Collier County, FL. You will perform patient assessments, develop care plans, provide skilled nursing interventions, educate patients and families, and collaborate with a multidisciplinary team to ensure high-quality, compliant home health care.

Qualifications

• Current and valid RN license in Florida.
• Minimum of 1 year of experience in a hospital or skilled nursing facility.
• Strong clinical skills and patient-centered approach to care.

Responsibilities

• Assess patients' physical, mental, and emotional health and vital signs.
• Develop individualized care plans including meds, wound care, and disease management.
• Educate patients and families on disease management and self-care.
• Provide direct care including wound care, medications, injections, and IV therapy.
• Collaborate with physicians and other healthcare professionals for coordinated care.
• Maintain accurate documentation of assessments and progress notes.
• Ensure compliance with HIPAA and Medicare regulations.
