Millennium Physician Group is seeking a Registered Nurse (RN) to join our Home Health Team in Florida. In this role, you will provide skilled, patient-centered home health care through assessments, care planning, clinical interventions, patient education, and collaboration with the care team while maintaining accurate documentation and compliance with regulatory standards.

You will assess, plan, educate, and treat patients in their homes, ensuring high-quality care and regulatory compliance.

Qualifications

• Active Florida RN license.
• Minimum 1 year of experience in a hospital or skilled nursing facility.
• Strong clinical and communication skills.
• Patient-centered approach to care.
• Reliable transportation to visit patients at home.

Responsabilités

• Conduct comprehensive patient assessments.
• Develop individualized care plans.
• Educate patients and families on disease management.
• Provide direct patient care including wound care, medications, injections, and IV therapy.
• Collaborate with physicians, therapists, and other healthcare professionals for coordinated care.
• Maintain accurate and timely documentation of patient assessments and progress notes.
• Adhere to HIPAA and Medicare regulations.
