Job summary

Providence at Home with Compassus in Portland, OR is seeking a registered nurse for full-time home health care. You will work with an interdisciplinary team to plan, implement, and evaluate skilled nursing care at patients’ homes across the region.

The role requires a state RN license, CPR and TB/Hepatitis compliance, and at least two years of clinical nursing experience, preferably in home health. Competitive compensation and a comprehensive benefits package are offered.

Qualifications

Graduate of an accredited college/university, Associate, Diploma, or Baccalaureate School of nursing.
Current, unencumbered State License as a registered Nurse.
Current Driver’s License.
Current CPR, negative TB screen and Hepatitis consent/declination.
Two years of experience as a registered nurse in a clinical care setting; home health preferred.

Responsibilities

Assess home health patients to identify physical, psychosocial, and environmental needs.
Case manage and provide clinical oversight to the interdisciplinary team and family.
Implement, develop, document, and revise the plan of care as needed.
Provide care with infection control measures to protect staff and patient.
Supervise and direct HHA/LVN to ensure quality and continuity of services.
