Elara Caring is seeking a Home Health Registered Nurse in Providence, RI and surrounding communities. This role focuses on one-on-one patient care, education, and building meaningful relationships with patients and families in home settings.

Qualifications

• Current state license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate/ Diploma/ Associate/ Baccalaureate).
• At least 1 year of nursing experience in a clinical setting.
• Home health experience preferred, but not required.

Responsibilities

• Complete initial and ongoing assessments to identify patient care needs.
• Provide skilled nursing care according to each patient’s plan of care and orders.
• Monitor patient conditions, report changes, and take appropriate action.
• Teach patients and families about care, safety, and managing health conditions.
• Coordinate with the care team to support safe, consistent patient care.
• Protect patient and staff privacy in accordance with HIPAA.
