Elara Caring is seeking a Home Health Registered Nurse to provide personalized, one-on-one skilled nursing care in patients' homes around Providence, RI. You will perform assessments, implement plans of care, educate families, and collaborate with a clinical team to ensure high-quality care.

Ideal candidates bring RN licensure and clinical nursing experience; home health is preferred but not required. This role offers compassionate patient relationships and meaningful, community-based impact.

Formación

• Current state license as a Registered Nurse (RN).
• Graduate of an accredited Nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• At least 1 year of nursing experience in a clinical setting.
• Home health experience preferred.
• Ability to lift/push/pull up to 50 lbs and assist patient transfers.
• Valid driver’s license or reliable transportation to travel between patient homes.

• Provide skilled nursing care according to plan of care and orders.
• Monitor patient conditions, report changes, and take appropriate action.
• Educate patients and families about care, safety, and managing conditions.

What You’ll Do

What You’ll Need

• Current state license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• At least 1 year of nursing experience in a clinical care setting.
• Home health experience preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
