Job summary

Aveanna Healthcare is seeking a Registered Nurse (Home Health) to provide one-on-one in-home nursing care in the Hernando and Citrus County communities. This full-time role follows a Thursday–Saturday schedule and requires an active RN license and at least 1 year of nursing experience.

You will assess patient needs, administer skilled nursing interventions, educate families, coordinate with the care team, and document care activities to support quality outcomes and safe, effective home health

Qualifications

• Active RN license in the state of practice.
• Minimum 1 year of clinical nursing experience.
• Current driver’s license and reliable transportation.
• CPR certification (or ability to obtain)

Responsibilities

• Provide one-on-one in-home nursing care that promotes recovery, safety, and independence.
• Conduct in-home assessments to evaluate physical, emotional, and environmental needs and translate findings into patient-centered care plans.
• Provide skilled nursing interventions including wound care, infusions, catheter care, medication management, postoperative care, and chronic disease management.
• Educate and empower patients and caregivers through clear instruction and support.
• Monitor patient progress, identify changes, and communicate with the care team to adjust plans and prevent hospitalizations.
• Coordinate care across providers to ensure continuity and high-quality outcomes.
