Job summary

Elara Caring is seeking a Registered Nurse Home Health to join our team providing right care at the right time and place. You will assess patients, perform ongoing evaluations, and administer nursing care per physician orders while upholding HIPAA and safety standards.

This role requires an active RN license and 1+ year of clinical nursing experience, with home health experience preferred. Candidates must be able to lift up to 50 pounds and have reliable transportation to travel among patient

Qualifications

• Current State license as a Registered Nurse.
• Graduate from an accredited nursing program (Certificate, Diploma, ASN or BSN).
• 1+ year of nursing experience in a clinical care setting.
• Home Health experience preferred but not required.
• Ability to lift/push/pull up to 50 pounds and transfer patients.
• Valid driver’s license and reliable transportation to travel to patient homes.

Responsibilities

• Assess patients to determine care needs.
• Complete initial and ongoing patient assessments.
• Protect patient and staff privacy under HIPAA.
• Provide nursing care per plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families on care and safety.
• Act promptly when patient conditions change.
• Contribute to quality and safe, continuous patient care.
