Job summary

Central Vermont Home Health & Hospice seeks a dedicated Registered Nurse to provide compassionate home health care within a Vermont territory. You'll deliver skilled nursing services, manage caseloads, and coordinate with physicians and community partners.

You must be a Vermont-licensed RN with drive and autonomy; prior home health or case management experience is valued. A reliable vehicle and VT driver’s license are required; orientation is provided.

Qualifications

• Vermont Licensed RN with a passion for home health care.
• Prior home health or case management experience preferred; training provided.
• Valid VT driver’s license and reliable vehicle with automobile insurance required.
• Ability to manage time and work independently.

Responsibilities

• Deliver high-quality nursing care to patients in their homes within a designated geographic territory and manage caseload to meet client needs.
• Foster collaborative partnerships with physicians and community providers.
• Establish goals of care and apply clinical and communication skills to help clients reach their goals.
