Job summary

University of Vermont Health - Home Health and Hospice seeks Registered Nurses to join the Adult Home Health team. You will provide skilled, compassionate care in patients' homes, collaborating with physicians, therapists, social workers, and administrative staff to keep patients safe at home.

The role offers autonomy within a supportive, mission-driven team and a Monday–Friday schedule with a rotating weekend day every 5–6 weeks. Training and professional development are emphasized.

Qualifications

• 1+ year of home health or public health nursing preferred; training available for motivated candidates
• Current RN licensure recognized by the State of Vermont required

Responsibilities

• Provide skilled, compassionate at-home nursing care to patients in their homes
• Collaborate with an interdisciplinary team to promote wellness and prevent hospitalizations
• Maintain high-quality patient relationships with focus on autonomy and safety
• Work Monday–Friday 8:00 a.m.–4:30 p.m. with rotating weekend coverage as described
