Job summary

Enhabit Home Health & Hospice is seeking an experienced licensed professional to administer skilled care at patients' homes. The role involves teaching patients and families, and coordinating with the health care team under the physician's plan of care.

A valid state license and CPR certification are required, with at least one year of licensed professional experience. Candidates should have basic technology skills and be able to operate a mobile device.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician's orders and the established plan of care (POC).
