Job summary

Enhabit Home Health & Hospice is seeking a licensed healthcare professional to administer skilled home health care. You will teach patients and families, follow physician orders, and implement the plan of care.

Ideal candidates hold a valid state license, have at least one year of licensed experience, and demonstrate basic technology skills with mobile devices. Medicare/home health experience is preferred; valid driver license and reliable transportation are required.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.
• Experience in Medicare home health or hospice is preferred.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
