Job summary

Unifi Healthcare is seeking a Licensed Field Nurse to deliver patient-centered home health care across client homes in our service area. You will provide comprehensive nursing services per the care plan during day shifts and travel between visits.

Responsibilities include vital signs, medication administration, documentation, and collaboration with the care team. A valid RN/LPN license and a reliable vehicle are required, with opportunities to support families and improve quality of life.

Qualifications

• Current, valid RN or LPN/LVN license.
• Reliable personal vehicle with valid insurance.
• Clear criminal background and negative pre-employment drug screening.
• Strong verbal and written English communication skills.

Responsibilities

• Provide compassionate personal care including bathing, dressing, grooming and hygiene support.
• Assist with mobility, transfers and safe ambulation for clients.
• Support toileting needs and catheter/ostomy care as required.
• Monitor vital signs, document intake/output, and administer medications as prescribed.
• Collaborate on individualized care plans and communicate changes to the Case Manager and physician.
• Perform light housekeeping and organize living spaces to promote safety.
