Job summary

Unifi Healthcare is seeking a Part-Time Registered Nurse or Licensed Practical Nurse to provide in-home, patient-centered care across assigned client plans. You will travel to client homes, monitor vital signs, administer medications, and document progress in the care plan while collaborating with the Case Manager and physicians.

Qualified candidates hold an active RN or LPN license, have a reliable vehicle with insurance, and can pass background checks and drug screening.

Qualifications

• Current, valid RN or LPN/LVN license.
• Reliable personal vehicle with valid insurance.
• Clear criminal background check and negative pre-employment drug screening.
• Strong verbal and written English communication skills.

Responsibilities

• Provide comprehensive, patient-centered care tailored to each client’s service plan.
• Monitor and record vital signs and administer medications as prescribed.
• Document intake and output and clinical progress in the care plan.
• Collaborate with Case Manager, physician, and interdisciplinary team.
• Travel to client homes and support daily living activities and safety.
