Job summary

Unifi Healthcare is seeking a Licensed Field Nurse to provide patient-centered care on the road, following each client’s care plan. You will promote independence, safety, and quality of life while delivering direct nursing services in clients’ homes.

Responsibilities include vital signs, medication administration, care-plan collaboration, and documentation. Travel to client homes is required, with flexible scheduling and weekend coverage as needed.

Qualifications

• Current, valid RN or LPN/LVN license.
• Reliable personal vehicle with valid insurance.
• Clear criminal background check and negative pre-employment drug screening.
• Strong verbal and written English communication skills.

Responsibilities

• Provide compassionate direct patient care per individualized service plan.
• Monitor vital signs and document intake/output.
• Administer medications as prescribed and report changes to Case Manager.
• Develop and implement individualized care plans with the team.
• Assist with housekeeping, errands, and transportation as needed.
• Maintain professional standards and boundaries; adapt to new responsibilities.
