Job summary

VitalCaring Group is seeking a Home Health RN to lead clinical care in patients’ homes across an assigned territory. You will manage the plan of care, coordinate with physicians and the care team, and drive outcomes from admission through discharge.

You’ll apply advanced assessment skills and education to reduce hospitalizations, improve recovery, and support long-term independence at home. Enjoy autonomy within a collaborative clinical framework.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• Strong clinical judgment and ability to think independently.
• Excellent communication and patient-centered approach.
• Ability to manage a caseload and prioritize effectively.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care, including medications and treatments.
• Coordinate across therapists, aides, and physicians as the case manager.
• Monitor progress and adjust plans based on changing conditions.
• Educate patients and families for disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR.
• Supervise Home Health Aides as needed.
• Participate in on-call rotation as appropriate.

Role Overview

As a Home Health RN, you are the clinical lead for your patients - managing the plan of care, coordinating across disciplines, and driving outcomes from admission through discharge.

You’ll combine advanced assessment skills, care planning, and patient education to reduce hospitalizations, improve recovery, and support long-term independence at home.

What You’ll Do

• Serve as the case manager, coordinating care across therapists, aides, and physicians

• Maintain timely, accurate documentation in the EMR
• Supervise and support Home Health Aides as needed

What You Bring

• Active RN license (or compact eligibility) + CPR certification
