Job summary

VitalCaring Group is seeking a Home Health RN to lead patient care in patients' homes within an assigned territory. You will manage the plan of care, coordinate across disciplines, and drive outcomes from admission through discharge.

You will apply advanced assessment, care planning, and patient education to reduce hospitalizations and promote long-term independence. Strong clinical judgment, time management, and 1+ year RN experience are essential.

Qualifications

• Active RN license in good standing.
• CPR certification required.
• Minimum of 1 year RN experience; Home Health preferred.
• Ability to think independently and make clinical judgments.

Responsibilities

• Conduct in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care including medications, treatments, and interventions.
• Coordinate care as case manager across therapists, aides, and physicians.
• Monitor progress and adjust care plans as conditions change.
• Educate patients and families to support disease management.
• Coordinate transitions and discharge planning to reduce readmissions.
• Document accurately in the EMR in a timely manner.
• Supervise Home Health Aides as needed and participate in on-call rotation.
