Zusammenfassung

VitalCaring Group is seeking a Registered Nurse (RN) to lead in-home care across a defined territory. You will conduct comprehensive assessments, develop patient-centered plans, and deliver skilled nursing care to improve outcomes in patients’ homes.

You’ll coordinate with interdisciplinary teams, manage cases, and document in the EMR with strong clinical judgment and communication. A field-based role with real autonomy and supportive leadership awaits.

Qualifikationen

• Active RN license (or compact eligibility)
• CPR certification
• Graduate of an accredited nursing program
• Minimum 1 year RN experience (Home Health, Med‑Surg, ICU, or Acute Care preferred)

• Conduct in home assessments to evaluate clinical status and goals of care
• Develop and manage individualized care plans with physicians and care teams
• Deliver skilled nursing care including medications, treatments, and interventions
• Coordinate care transitions and discharge planning to reduce readmissions
• Maintain timely documentation in the EMR
