Job summary

Optum’s Louisiana HomeCare Egan is hiring a Registered Nurse - Home Health to deliver high-quality nursing care in patients' homes, in collaboration with physicians and care teams. You will assess, plan, and help coordinate the total plan of care for optimal patient outcomes.

This role requires applying clinical expertise, supervising aides, and maintaining accurate documentation while adhering to state laws and agency policies. It is a full‑time position with competitive pay and benefits.

Qualifications

• Current and unrestricted RN licensure in state of practice
• Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation
• LA state-specific requirements: 1+ year RN clinical experience; unrestricted licensure

Responsibilities

• Initiates and revises the plan of care with the physician.
• Supervises home health aides and licensed nurses, providing instruction.
• Performs required supervisory visits and ensures compliance with regulations.
• Completes OASIS assessments and medication reconciliation accurately and timely.
• Coordinates care with physicians, agency staff, patients, and families to ensure continuity.
