Job summary

Optum's Mederi Caretenders, part of the LHC Group, seeks a Registered Nurse for home health in Florida. You will deliver and coordinate nursing care in patients' homes, align with physicians, and ensure compliance with laws and policies while supporting families and communities.

You will develop care plans, supervise aides, document visits, and collaborate with the care team to achieve strong patient outcomes. We offer comprehensive benefits and growth within the Optum family.

Qualifications

Current RN license in state of practice.
Current Driver's License and access to reliable transportation.
CPR certification or ability to obtain within 90 days of hire.

Responsibilities

Initiates, develops, implements, and revises the plan of care with physician and health care team.
Supervises home health aides and licensed staff; provides instruction and assigns tasks per regulations.
Documents patient visits and ensures timely data entry per policy and payer requirements.
Provides supervisory visits as required and coordinates care delivery.
Communicates with physicians, patients, and family to ensure quality care and service excellence.
Participates in on-call and weekend rotations as needed.
