There For You Home Care Services in Waterbury, CT is seeking a compassionate Registered Nurse to deliver skilled nursing care in patients' homes. You will assess needs, develop care plans, administer treatments, and coordinate with physicians and the interdisciplinary team to support recovery and safe home living.

The role requires CT RN license, valid driver's license, and experience with OASIS. Strong EMR documentation, supervision of LPNs/HHA, and patient education are essential;

Qualifications

• Current, unrestricted RN license in Connecticut.
• Valid driver's license and reliable transportation.
• Experience completing OASIS assessments.
• Graduate of an accredited nursing program.
• Current BLS/CPR certification.
• Ability to work independently with strong clinical judgment.
• Proficiency with EMR systems and general computer skills.
• 1+ year in home health preferred.

Responsibilities

• Perform in‑home patient assessments and start/recertify care plans.
• Develop and revise individualized plans of care with the physician team.
• Provide skilled nursing care including wound care, meds, IV therapy, and catheter/ostomy care.
• Educate patients and caregivers on meds, disease processes, and self‑care.
• Coordinate care with physicians, therapists, social workers, and home health staff.
• Supervise LPNs/LVNs and home health aides with supervisory visits.
• Document care accurately in the EMR per policy and regulations.
• Ensure HIPAA compliance and regulatory standards.
• Complete OASIS documentation to support coding and reimbursement.
• Participate in case conferences and on‑call rotations; manage caseload.
