Job summary

UniversaCare’s There For You Home Care Services seeks a compassionate RN to deliver skilled nursing care in patients' homes across Waterbury, CT and surrounding towns. You will assess needs, develop care plans, administer medications, and coordinate with physicians and the interdisciplinary team.

This role requires OASIS experience, strong clinical judgment, and independent work in a home-based setting. A CT RN license, BLS/CPR, and EMR proficiency are essential, with opportunities for growth

Qualifications

• Current, unrestricted CT RN license.
• Graduate of an accredited nursing program.
• Experience completing OASIS assessments.
• BLS/CPR certification.
• Proficiency with EMR systems.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct in-home patient assessments, including OASIS at start of care and discharge.
• Develop and revise care plans with physicians and interdisciplinary team.
• Provide skilled nursing services: wound care, meds, IV therapy, catheter/ostomy.
• Educate patients and caregivers on meds, disease processes, safety, self-care.
• Coordinate care with physicians, therapists, social workers, aides.
• Supervise LPNs/LVNs and aides, with supervisory visits.
• Document care in EMR per agency policy and regulatory standards.
• Ensure compliance with Medicare/Medicaid requirements and HIPAA.
• Complete OASIS documentation to support coding and reimbursement.
• Manage caseload and plan daily visits across service area.
