Job summary

Covenant Health is seeking a Registered Nurse for HomeCare to provide professional nursing care to homebound patients. The RN-Home Health coordinates care planning with a Clinical Supervisor and other disciplines and reports to the Nursing Clinical Supervisor.

The role involves performing assessments, medications administration, care planning, and direct patient education in a home setting. Local travel is required; opportunities include growth and leadership development within Covenant Health.

Qualifications

• Graduation from an accredited school of nursing.
• One year of professional experience in hospital or prior home health experience required.
• Must have and maintain State of Tennessee RN license; CPR required; valid Tennessee driver’s license (Class D) and state-mandated minimum insurance coverage.

Responsibilities

• Reviews patient records, determines home care needs, develops care and discharge plans, and coordinates with other disciplines.
• Performs cardiovascular, pulmonary, neurological, GI, skin, intake/output, and pain assessments.
• Administers medications and documents administration; checks allergies and MD orders.
• Acts as liaison between MD and patient/family; informs MD and staff of changes in patient needs.
• Supervises LPNs and Home Health Aides; facilitates orientation of new employees.

Education
