Job summary

Beth Israel Lahey Health is seeking a Home Health Registered Nurse to plan, organize, and direct home care services, bringing community health education to patients in their homes and communities in MA/NH.

The role requires initial assessments, care plan development, medication administration, and supervising home health aides, with HIPAA compliance and EPIC documentation. It offers competitive pay and a comprehensive benefits package.

Qualifications

• Active MA and NH RN licenses; willing to obtain the other within 3 months of hire.
• Active American Heart Association BLS certification.
• Associate’s Degree or BSN from an accredited school of nursing.
• At least 1 year of direct care nursing experience; OASIS and/or EPIC experience preferred.

Responsibilities

• Complete and document an initial assessment of patient and family to determine home care needs.
• Initiates the plan of care and revises as patient status changes.
• Administers medications and treatments as prescribed by the physician.
• Instruct, supervise, and evaluate the home health aide care every two weeks.
• Identify discharge planning needs as part of the care plan.
