Job summary

Elara Caring in Texas is seeking a Registered Nurse Home Health to join our patient-centered team. In this role you’ll assess patients, perform initial and ongoing assessments, and administer care per the plan of care and physician orders.

You’ll monitor patient conditions, educate families on care and safety, and take action as conditions change. RN license and 1+ year clinical nursing experience required; home health experience preferred.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (certificate to BSN).
• 1+ year of experience in a clinical care setting.
• Home Health experience preferred but not required.
• Ability to lift up to 50 pounds and travel between patient homes.

Responsibilities

As a Registered Nurse Home Health , you’ll contribute to our success in the following ways:

• Assessing patients to determine their care needs.

• Completing initial and ongoing patient assessments.

• Protecting patient and staff privacy in accordance with HIPAA.

• Providing nursing care according to the plan of care and physician orders.

• Monitoring patient conditions and reporting changes to the care team.

• Educating patients and families on their care and safety.

• Taking appropriate action when patient conditions change.

• Helping ensure quality, safe, and continuous patient care.

What is Required?

• Current State license as a Registered Nurse.

• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.

• 1+ year of experience as a nurse in a clinical care setting.

• Home Health experience is preferred, but not required.

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
