You will case manage and provide clinical oversight to the interdisciplinary team, coordinate visits, and empower patients to stay independent.

Requirements include Alabama RN license, two years of RN experience (home health preferred), driver’s license, CPR/BCLS, and a caring, inclusive approach.

Qualifications

• Alabama RN license, active in good standing.
• Two years of RN experience; home health experience preferred.
• Current driver’s license and CPR/BCLS certification.

Responsibilities
