Job summary

Advocate Health is seeking a part-time Registered Nurse in Sheboygan to provide care visits as part of an interdisciplinary team. The schedule includes Friday/Saturday/Sunday 8am-4pm or Saturday/Sunday 8am-8pm with an on-call rotation; weekend holiday coverage may apply.

Qualifications

• Completion of an accredited nursing program and RN licensure in the practicing state.
• BLS for Healthcare Providers certification.
• Ability to develop care plans and collaborate with interdisciplinary team.

Responsibilities

• Performs scheduled nursing visits and updates plan of care.
• Communicates with physicians and RN Case Manager about changes.
• Maintains patient records per guidelines and participates in discharge planning.
