Job summary

Team Select Home Care of Indiana, LLC is seeking a Registered Nurse (RN) for private duty home health across Anderson, IN area. You will deliver one-on-one nursing care to medically complex pediatric and adult patients in their homes, following physician orders and the care plan.

The role offers flexible scheduling (PRN, part-time, full-time) and weekly pay, with opportunities for professional growth and supportive supervision. Active Indiana RN license and CPR are required.

Qualifications

• Graduate of an approved school of professional nursing and currently licensed in the state(s) practicing.
• Active Indiana RN license required.
• CPR certification required.
• Ability to meet health requirements and maintain licensure.

Responsibilities

• Develops, delivers/implements, revises, and evaluates individualized patient-centered care plans under physician supervision.
• Provides case management to an assigned group of patients and ancillary staff.
• Understands and submits required data collection sets and other documentation as requested by state/federal payers within required timeframes.
• Makes initial evaluation visits and reevaluates nursing needs per state/payor requirements.
• Educates patients and families on home health strategies and coordinates care between patients, families, and healthcare providers.
