Team Select Home Care is seeking a compassionate Registered Nurse to join our private duty home health team in Indiana. You will provide skilled nursing care to medically complex children and adults in patients' homes, coordinating with physicians, therapists, and families to ensure safe, effective care.

You'll document visits in our EMR, participate in case conferences, and engage in ongoing education and support for families.

Qualificações

• Active RN license required
• CPR certification required
• Clinical experience preferred but welcoming new grads

Responsabilidades

• Develop, revise, and evaluate patient-centered care plans under physician supervision
• Provide case management for assigned patients and staff
• Submit data collection and documentation per state/federal requirements
• Educate patients and families on home health care
• Document visits accurately and communicate changes to the care team
• Contribute to case conferences and collaborate with the clinical supervisor
