Team Select Home Care is seeking a Registered Nurse (RN) to deliver compassionate private duty nursing in patients’ homes, focusing on medically complex children and adults. You’ll coordinate with physicians and therapists, monitor progress, and educate families for effective home care.

Qualifications

• Active RN license required.
• CPR certification required.
• Clinical home care experience preferred but open to new grads.

Responsibilities

• Provide one-on-one in-home nursing for medically complex pediatric and adult patients.
• Coordinate care plans with physicians and therapists.
• Monitor patient progress and update plans as needed.
• Educate families on home care techniques and safety.
• Document visits in the EMR and maintain accurate records.
• Participate in case conferences and collaborates with the care team.
