The role requires an active Florida RN license, at least one year of hospital or skilled nursing facility experience, reliable transportation, and a compassionate, patient-centered approach.

Responsibilities

• Patient Assessment: assess patients’ physical, mental, and emotional health
• Care Planning: develop individualized care plans

• Collaboration: work with physicians and other professionals for coordinated care
• Documentation: maintain accurate progress notes and assessments
